Я учусь в области healthcare. Это для меня новое русло, поэтому буду очень признателен, если кто-нибудь подкинет мне небольшую реальную историю. А суть такова:

Think of a time when you or someone you know has had an encounter with the health care system. Give a brief synopsis of that experience and comment on how the encounter did or did not demonstrate fragmented versus integrated health care. If your chosen encounter demonstrates fragmented care discuss the barriers to care continuity and how the system could be improved. If your chosen encounter demonstrates continuity of care discuss what system factors contributed to this.

Example of a fragmented care would be the case you need to see a specialist (not your regular primary care), but (1) not able to find anyone within the same hospital/clinic or (2) your health insurance does not cover available specialists, so you have to go someplace else. As the result there no clear communication and record availability between doctors (time constraints, protection of clinical info, laziness, etc), so you end up getting care form 2 different doctors, you get prescribed medications which potentially might interact, and if not cause damage, just nil each other.

Think of elderly relative seeing psychiatrist for depression and PCP for high blood pressure, diabetes and pain. It’s clearly proven, that some blood pressure meds might cause depression, pain definitely both cause and get worsened by depression, and psychotropics (not all, but some) might increase risk factors for medical illnesses, but in many instances are also indicated as a part of pain treatment. So, if drs are aware of each others existence and treatments, they could coordinate more effectively, decreasing number of medications and improving outcomes. If not => increased risk of potentially harmful med interactions, etc.

Keep in mind, that patients not always inform drs of seeing specialist (in case of psychiatrist - fear of stigma, shame), or any other professional as well as they feel it undermines a physician.

Overcoming:
Ideally - “Весь мир насилья мы разрушим до основанья, а затем…”
In reality:

  • better screening (ask pts to bring all meds and review, call pharmacies or insuarance company to get a list of all meds, some states mandate PCPs to share yearly physical results with narrow specialists, if requested, etc.)
  • electronic record keeping
  • allowing drs to have more time for face-to-face with patients
  • insurance companies encouraging/rewarding MDs for better outcomes (I heard gossips, but how it would look in practice…???)

In my experience:

  1. Unpredicted clinical schedule for most medical facilities. Usually, a patient has no ability to complete treatment plan.
  2. Difficulty with the sharing/transferring of patient’s medical history between facilities. Keep in mind, “good” physician has to read history before appointment :slight_smile:
  3. Of course, medical errors! Malpractice, wrong medications, wrong diagnoses, wrong transferring, wrong billing…

Ну это когда вам надо сдать анализы проконсультироваться с доктором и по решению сделать операцию, но время ограничено свежестью результатов к примеру MRI. Но ввиду того, что аппоинтментов у дохтора нет вам не удаётся всё сделать вовремя и надо начинать сначала MRI-aнализы-интерпретация-пре-операционный чеклист-и только потом ВОЗМОЖНО операция. Ситуация утрированная, обычно всё ещё более запутаннее - сюда прибавляются такие переменные как “использование медикаментов” :slight_smile:

Не только. Нет единых электронных стандартов и возможности доступа, потерянные файлы, бешенные суммы за копии (я видел 3$ за страницу), географическое положение,частные клиники на контрактах у госпиталей, да и лень многим просто…

Обучение в идеале должно представлять из себя оттачивание и применение ваших академических знаний.

Купите толковые книги, ProQuest не помешает, LexusNexus или как там его, и ваша университетская онлайн лайбрары…

Вы ж учитесь для себя - корка нужна - но в большинстве бесполезна…

Green card holders pay less tuition for university and college, referred as “in-state” tuition or “resident” tuition. Savings are considerable and in most cases, they are 3 to 4 times lower than what foreigners pay.

Вопрос: как это работает и выражается в ден.эквиваленте. Интересуют коледжи healthcare (nurse) в NJ. И чего может стоить диплом фельдшера частного укр.коледжа после эвалюации?
Интересуюсь для своей сестры, которая приедет по ГК в Н.Джерси в мае.

Она закончила коледж на Украине в прошлом году. Работала недолго медсестрой, сейчас не работает, только учит язык и тд.
Если о себе, то я тоже закончил медучилище. Но сейчас с медициной не связан. Вообще.:frowning:
